If you are less than 35 have been trying to get pregnant for over year, or over 35 and have been trying for more than 6 months, it is time to seek out the advice of an infertility specialist.
Both the husband and wife should plan on attending the initial consultation with the physician. The woman will be examined and asked about her puberty, menstrual history, previous pregnancies, medications, and sexually transmitted diseases. Any prior infertility evaluations, surgeries, and medical treatments should also be discussed. A physical examination will include evaluation of the pelvis to detect any possible cervical, uterine or pelvic issues.
The husband may be asked about general health, medications, genital issues, and smoking, drug and/or alcohol abuse. The physician may suggest that a physical be performed on the husband. If a semen analysis has not been performed in the last 12 months, then one will be ordered.

A semen analysis should be performed on all couples that are having an infertility evaluation. This will involve collecting a semen sample in a sterile specimen cup provided by your physician. It is generally recommended that you abstain for at least 2 days, but no more than 5 days prior to your test. You can download detailed instructions here. We realize that there may be some reluctance to collect a sample in our facility. Therefore, we allow men to collect at home and deliver the sample to our facility within 45 minutes. It is important that the sample stays as close to body temperature as possible. Exposure to temperatures hotter or cooler than body temperature could kill the sperm.

Dr. Pirkle will evaluate your sample as soon as it is received. The amount of semen present is measured and the color is noted. A typical ejaculate is only about a teaspoon or less, so don’t worry about filling up the cup! It is important, however, that ALL of the semen make it into the cup, otherwise the count may not be accurate. The color of the semen should be cloudy white.
A drop of semen is then placed on a counting slide, which is used to determine the number of sperm present and the % of sperm that are moving. The % of sperm stuck together (agglutination) and how well the sperm move forward are also noted. A drop of sperm is also smeared on another slide and allowed to dry. This slide will be stained so that the shape of the sperm (morphology) can be visualized under high magnification.

Because there can be variation from sample to sample, ti is recommended that if an abnormality is found a repeat analysis should be carried out 2-3 months later.
Other tests that may be performed include:
Sperm antibody assay – to detect the presence of antibodies in the semen that attack the sperm and prevent them from fertilizing the egg.
Fructose – performed when no sperm are present
Hyaluronan Binding Assay – a measurement of ability of sperm to bind to an egg.

The HSG, or HysteroSalphingoGram, gives the physician information about the shape of the uterine cavity and whether or not the fallopian tubes are open. This will generally be performed in the Radiology Dept of a hospital. An instrument is inserted through the cervix and dye is injected into the uterus. The radiologist will then take a series of S-rays. If the fallopian tubes are open, the images will show the dye flowing through the uterus and out the fallopian tubes. If there is a blockage in one or both tubes, the image will indicate that as well.
